1. The required annual physical and health history cannot be filled out online as it must be completed by the parent/guardian and physician.

2. All other required paperwork, not included in the physical folder, will be done electronically. The parent/guardian will be asked to provide information such as; home phone numbers, cell numbers, e-mail addresses, primary physician, insurance policy information, and other pertinent information. Please make sure that you have this accurate, pertinent information available when filling out your forms electronically.

3. Remember, that to submit online appropriately, you and your child must fill out and sign in all required spaces (Space provided indicates required information). Forms will not be accepted electronically without all required information being completed.

4. To access the required participation forms for Splendora ISD, hold your cursor over the “Electronic Forms” tab. You will get a drop list of all required forms. All forms under the tab must be completed in order to be eligible to participate.

5. To begin, click the form name and fill out the information requested. Parent/guardian must have child present along with the student’s school ID number available to complete forms. If you do not know or do not have an answer to one of the questions, please respond with N/A. Not Applicable, or None. Please do not leave blanks as the form will not be accepted.

6. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the “Refresh” icon next to the signature box. If you are using an iPad, you may create a signature by holding your finger to the screen and signing your name or by use of a stylus.

7. Once you have completed all the information on each page, you will have the opportunity to print a personal copy of the document.

8. You will receive a confirmation by e-mail once the document(s) have been reviewed by the Splendora ISD Athletic training Staff.

1. La evaluación física de pre-participación y el expediente médico no puede ser completado electrónicamente. Este formulario debe de ser completado por el médico y el padre o encargado del estudiante.

2. Los demás formularios deberán ser completados electrónicamente. El padre o encargado deberá proveer información como dirección de casa, teléfono, correos electrónicos, nombre de la aseguranza médica, nombre del doctor y cualquier otra información que necesitemos.

3. No se olvide que para completar este proceso Ud. y su hijo/a deberán completar y firmar los formularios electrónicamente.

4. Para acceder estos formularios de Splendora ISD., Ud. deberá mover al ratón a la sección llamada “Electronic Forms” donde vera todos los formularios que tendrán de ser llenados completamente para poder participar en atletismo.

5. Para comenzar este proceso, elija el formulario con el cual va a comenzar. Su hijo/a necesitara su número de identificación para el estudiante (student ID#). Por favor no deje ningún espacio blanco. Si Ud. no sabe la respuesta a una de las preguntas, escriba N/A (no aplicable).

6. Para firmar el documento, ponga el ratón dentro de la casilla y firme con el ratón. Si esta usando un iPad, podrá firmar usando su dedo.

7. Una vez que haya completado toda la información en cada página, tendrá la oportunidad de imprimir una copia para sus archivos.

8. Ud. recibirá una confirmación en su correo electrónico después que el departamento de atletismo de Splendora ISD haya revisado los documentos recibidos.
